After downloading one of the forms below, email, fax, or mail them to NCF at the contact information on the form (or on the "Contact Us" link on the left bar).

• Authorization to Add or Remove Advisors – to give others access to your Fund information.

 
Wire Notification Form

• Wire Notification Form – to notify NCF that you are wiring money to your Fund at NCF
   

Giving Securities

• Letter of Authorization for Stock Transfers – to transfer stock on deposit in your brokerage account. If your shares are held at Computershare, please contact Computershare and request your shares in certificate form. Computershare will send you a certificate, and then use the Certificate Release Request and the Irrevocable Stock Power Form below to transfer your stock certificate to NCF.
   
• Letter of Authorization for Mutual Fund Transfers – to transfer mutual funds to NCF.  Select the institution you're transferring from:
  • Fidelity - this form may also be used to transfer mutual funds if they are held at a firm other than the ones listed. Check with your broker to make sure the mutual funds are transferable. If they are not transferable, contact us at 800.681.6223.

Testamentary giving forms

• Testamentary Gift Information – explains the process for naming NCF as the beneficiary in your will/estate 
   
• Giving Fund Succession Plan Letter of Advisement – allows you to recommend to NCF where your NCF Giving Fund balance is distributed upon your death ... (For NCF Legacy Fund testamentary advisement, click on the "Legacy Fund Application" link above.) 
   

Grant recommendation forms

• Domestic Grant Recommendation Form – for grants to U.S.-based charities (though recommending grants is free and fast on NCF's Donor Website)
